Recreational Divers
If you have been asked to see a Doctor before going on a dive course or holiday Dr Kohn will happily see you for a consultation to assess you as a healthy diver. Please bring the internationally recognised PADI Medical Statement form which can be downloaded from the Padi Website.
HSE Diver Medicals
Dr A Kohn is approved by the Health and Safety Executive (HSE) to provide diving medicals for professional divers. Your first HSE Medical requires a full blood count and an audiogram. Each subsequent medical is repeated annually and at a lower cost. Each medical lasts approximately 45minutes – 1 Hour.
In preparation for your medical;
- For your first medical please download and complete this form regarding your medical history. The form must be countersigned by your GP. Please bring it to your appointment or have the GP post, email or fax it back to us.
- Annual medicals require that you bring a copy of your most recent HSE medical with you. We keep copies of all medicals that are carried out at the surgery, so if you saw Dr Kohn for your last medical, no need to worry.
- The medicals include an exercise test so please dress appropriately.
- Please bring your dive log book.
- Renewal medical certificates are issued at the end of your medical. First HSE medicals require 2 days for the blood results to be returned and then we will forward the medical to you and the relevant certificate to the HSE.
