What To Bring To Your Medical
- Form 1007 (one per person) – please fill out Sections A & B (p.1-5) before coming to your appointment. The form can be downloaded from the New Zealand Immigration Website
- A passport photo (an additional, identical picture, is needed for the chest X-Ray section)
- Passport
- Glasses (if you wear them)
- Any significant medical history notes
- Red books (children only)
- A urine sample will be required for everyone aged 5 and over upon arrival
- The medical takes approximately 30 minutes per person
We will provide you with details of how and where to get your chest X-Ray completed. Remember that only people aged 11 years and over will require a chest X-Ray.
